Fixes

An issue was identified where under certain conditions (combination of total certificate volume and concurrent operations) the SQL query handling the deactivation of old records would time out and fail.
CABs performing full replacement uploads were susceptible to this bug, which could cause the creation of duplicate certificates as the creation of new records would proceed.
The query, related APIs and file queuing service have all been hardened to prevent this from reoccurring.
